Patients with HMO, PPO, POS, or other managed care plans are responsible to follow the rules and guidelines provided by that plan. Some plans require verbal referrals, referral forms, or referral numbers for services provided by a specialist which the patient must obtain from their primary care physician prior to their visit. We will need your insurance name, ID number, Group number, and the Customer Service phone number shown on your insurance card. Please bring your insurance cards including any pharmacy cards to your visit.
